Job Title: Home Care Field Care Supervisor

Job Specification and Responsibilities
- Responsible for a group of customers and Care Workers within the local community, ensuring their best interests and safeguarding their wellbeing.
- Ensuring our customers are receiving the care and support they expect and deserve.
- Field-based role, liaising with customers directly both at the commencement of the service and throughout its duration.
- Conducting assessments and regularly reviewing how the service is delivered.
- Supervising and managing Care and Support Workers, ensuring adherence to Concept Care’s high standards and CQC guidelines.
- Working closely with the Home Care Manager and Coordinators to plan services effectively.
- Holding regular team meetings and one-to-one meetings with Care Workers to discuss progress and training.
- Maintaining professional relationships with customers and Care Workers to ensure a high-quality service.
- Carrying out service user assessments and re-assessments.
- Performing spot checks and quality monitoring.
- Out-of-hours work may be required.
- Must be punctual and adhere to agreed working hours.
- Covering care calls in emergency and ad hoc situations (following company policy on logging in service users’ homes for care or supervision).
- Flexible working hours.
- ‘On-call’ duties on a rotational basis to support business needs.
- Adhering to set departmental budget constraints.
- Undertaking any other duties required to support the growth and development of the business, including covering care calls when required.

- Good typing skills with accurate spelling and grammar.
- Computer literate, particularly in Word and Excel.

Qualifications
- Health and Social Care Level 2 & 3 or equivalent (or working towards Level 3).
