About the Roles

We are looking for experienced and motivated Specialist Clinical Pharmacists to join a rotational clinical pharmacy service.

These are excellent opportunities for pharmacists looking to develop their specialist clinical skills across a range of areas while providing high-quality pharmaceutical care to patients.

Successful candidates will rotate across areas including Oncology/Haematology, Frailty and Older Person Care, and Urgent and Emergency Services, while also supporting wider clinical pharmacy services across the Trust.

You will work closely with multidisciplinary teams to optimise medicines, improve patient safety and ensure continuity of pharmaceutical care from admission through to discharge.

Key Responsibilities

• Provide specialist clinical pharmacy services across designated rotational areas.
• Review prescriptions and develop appropriate pharmaceutical care plans.
• Undertake medicines reconciliation and drug history reviews.
• Participate in consultant ward rounds and multidisciplinary team meetings.
• Provide specialist medicines advice to patients, carers and healthcare professionals.
• Support safe and effective medicines optimisation.
• Identify, manage and report medicines-related errors and near misses.
• Contribute to clinical audits, quality improvement and service development.
• Support the development of clinical guidelines, policies and SOPs.
• Develop and utilise independent prescribing skills within appropriate clinical areas.
• Supervise and support junior pharmacists, trainee pharmacists and pharmacy technicians.
• Deliver education and training to pharmacy staff and other healthcare professionals.
• Contribute to weekend and on-call pharmacy services.

Essential Requirements

Applicants should have:

• Master’s Degree in Pharmacy
• Current GPhC registration
• Diploma in Clinical Pharmacy Practice or equivalent experience
• General-level Foundation Framework competency training
• Significant clinical pharmacy knowledge and experience
• Significant hospital pharmacy experience
• Knowledge of national medicines and pharmaceutical practice guidance
• Specialist knowledge and experience within a clinical specialty
• Excellent communication and interpersonal skills
• Strong organisational and time-management skills
• Ability to interpret complex clinical information
• Ability to work independently and as part of a multidisciplinary team

Applicants should be confident communicating with patients and healthcare professionals, interpreting complex clinical information and making appropriate pharmaceutical decisions.

Working Pattern

These are full-time positions, primarily providing a weekday service with participation in weekend and on-call duties. On-call pharmacist duties are undertaken from home, with calls initially screened by the Trust Clinical Site Management Team.
