District Nurse (Preston)
Key Responsibilities:
- Conduct comprehensive assessments of patients’ health needs in their homes or community settings.
- Develop and implement individualized care plans based on patient assessments and in consultation with the multidisciplinary team.
- Provide skilled nursing interventions, including wound care, medication management, and symptom control.
- Educate patients and their families on disease management, treatment plans, and self-care techniques.
- Coordinate with other healthcare professionals, such as physicians, therapists, and social workers, to ensure continuity of care.
- Monitor and evaluate patients’ progress, adjusting care plans as necessary to achieve optimal outcomes.
- Advocate for patients’ needs and rights, ensuring they receive appropriate and timely healthcare services.
- Maintain accurate and up-to-date documentation of patient assessments, care plans, and interventions.
- Participate in ongoing professional development and training to enhance clinical knowledge and skills.
- Adhere to all relevant policies, procedures, and regulatory standards to ensure safe and effective care delivery.
Qualifications:
- Bachelor’s degree in nursing.
- Minimum 12 months of NHS experience.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
Physical Requirements:
- Ability to travel to patients’ homes and community locations.
- Ability to lift and move patients as needed.
