Mental Health Care Coordinator (West London)

Take on the role of Care Coordinator, where you will be responsible for a diverse caseload of service users, primarily aged 65 years and older, who present complex mental health and social care needs. Your mission is to provide unwavering support to clients, their devoted caregivers, and families, all while delivering top-tier healthcare within the framework of the Care Programme Approach. This role offers a competitive hourly rate, ranging from £24.00 to £30.00 per hour.

Qualifications & Training

Essential

• Car Driver
• A working knowledge and understanding of raising safeguarding concerns
• NMC Registration RMN/RNLD
• BSc Mental Health Nursing or Equivalent

Desirable

• Practice Assessor Trained
• • Phlebotomy Trained
• Experience of managing risk within a complex caseload.

Experience 

Minimum 6 months NHS experience
