Care Coordinator (London)

Concept Care Solutions is a GOOD RATED CQC registered Domiciliary Care Provider providing care to vulnerable young adults & elderly clients in the Islington Area.

We are currently on the search for an experienced Domiciliary Care Coordinator to Join our team in Islington.

The Role:

• Will be an experienced Care Coordinator who has managed 700plus hours a week
• Have a NVQ/QCF Level 3 or working towards in health & social care
• The Care Coordinator will have the ability to manage a cohort of care staff and their rotas
• The Care Coordinator will take part in out of hours on call rota
• The Care Coordinator will have the ability to take on new packages and clients to grow the business
• The Care Coordinator will be proactive in completing risk assessments, care plan assessments and providing hands on care if needed.
• Ideally have used CAREFREE & PASS systems or something similar.
• The Care Coordinator will have a good working knowledge of Microsoft Office including Word, Excel & Outlook. Experience using Share Point is a bonus!
